Task Risk Assessment Template
A focused risk assessment for a single task or activity. It captures the people, equipment, materials and substances involved, the hazards and who is exposed, a structured set of prompts to check the task has been thought through, and the additional controls required.
The Management of Health and Safety at Work Regulations 1999 require a suitable and sufficient assessment of the risks from work activities. This form gives you a practical, task-level way to meet that duty, and supports the hazard identification and operational control requirements of ISO 45001.

- The task or activity, its location, and the people, equipment, materials, substances and infrastructure involved
