What's Included in this Risk Assessment Template
The template uses a straightforward inherent risk / residual risk approach, supported by a built-in risk rating matrix to help assessors consistently evaluate likelihood and consequence. The form captures all the key information required for a compliant and auditable risk assessment, including the activity or task being assessed, associated hazards, potential consequences, control measures currently in place, and the resulting residual risk rating. Risk-ratings are entered using a pre-populated combo box by clicking the relevant entry from the list to ensure consistent and valid risk ratings are entered.
The file has 2 versions; F-HS20A and F-HS20B. The A version is the full risk assessment form and the B version also includes a listing of hazard categories, hazard symbols and PPE (personal protective equipment) icons, allowing assessors to easily add these symbols to visually communicate hazard types and required protective equipment within the completed assessment.
Header styles are colour-coded to match risk ratings, making completed assessments clear and easy to read at a glance.
An additional appendix file is also supplied with all the symbols.
