This form template can be used to provide an overview of safety and other arrangements in place on a work site while also allowing any other hazards and risks identified at the site before starting work to also be reviewed and detailed.

This form also has a section for ‘Review of Hazards and Confirmation that safe to proceed with work’ which requires confirmation that all hazards have been identified, that effective controls are in place and that safe to commence work.

There are 2 versions of the form, version 2 also includes a side induction and acknowledgment section to be signed-off by site staff.

The form template layout and content can be easily modified as required.

This version of the policy template is designed for small companies who wish to have a documented H&S policy and simple overview of arrangements. This version of the policy is not suitable for ISO 45001 compliance.
