General and Fire Risk Assessment Template

A combined assessment covering both general workplace risks and fire safety in one document. It records each activity, hazard and consequence and each fire safety consideration, with inherent and residual risk ratings either side of the controls, and confirms whether residual risk is at an acceptable level.

It brings together the risk assessment required by the Management of Health and Safety at Work Regulations 1999 and the fire risk assessment required by the Regulatory Reform (Fire Safety) Order 2005. This form gives you both in one place and supports the hazard identification and emergency preparedness requirements of ISO 45001.

Built for Both Duties and ISO 45001

The assessment supports the risk assessment and emergency preparedness requirements of ISO 45001 (clauses 6.1.2 and 8.2):

  • General activities, hazards and consequences assessed with inherent and residual risk ratings
  • A dedicated fire safety section covering the fire considerations for the premises
  • Control measures in place recorded against each hazard and consideration
  • An explicit confirmation of whether residual risk is acceptable, with actions where it is not

What's Included in this General and Fire Risk Assessment

The form runs from the general assessment through the fire section to sign-off:

  • Header - completed by, date completed, number, the location, task or activity, and who is affected or at risk
  • General risk assessment - numbered activities, hazards and consequences with inherent risk, controls in place and residual risk
  • Fire safety section - numbered fire safety considerations rated the same way
  • Acceptability confirmation - a statement that residual risk is acceptable, or the actions required if not
  • Additional controls and notes - further actions and opportunities to reduce risk

Who Needs a General and Fire Risk Assessment?

Any organisation that wants its general and fire risk assessments in a single record - especially smaller premises where the two overlap. It suits SMEs across every sector and is a core record for any business working towards or maintaining ISO 45001 certification.

This form is part of the ISO 45001 Toolkit - risk assessments, registers, procedures and guidance for managing health and safety in one download - and is included in every alphaZ ISO toolkit that covers ISO 45001.
