A number of forms for assessing, planning and monitoring the use of vibrating equipment where there is a risk of harm from Hand-Arm vibration.
Hand-arm vibration is vibration transmitted into workers’ hands and arms. This can come from the use of hand-held power tools and long-term or excessive exposure can cause Hand–Arm Vibration Syndrome (HAVS) and other health issues.
Forms included;
- F-HS45A HAVS Assessment - this form can be used as part of the job planning process and details the planned activities, section for listing vibrating equipment required including a risk rating and max safe operating time and then a section for summarising the work using vibrating equipment with max trigger times. There's then a review of hazards with various checks listed such as ensuring all operatives are aware of ELV and confirmation that safe to proceed & comments where notes and actions can be added
- F-HS45B Hand-Arm Vibration Use Record - this form can be used by a single worker to log their use of vibrating equipment over a period of time with date, equipment used, work completed and vibration risk rating, max safe operating time and total trigger time all logged.
- F-HS45C Hand-Arm Vibration Use Record - this form as as per F-HS45B but does not have a name at the top with name instead included in the table so the register can be populated with the data of multiple workers. This form is suitable for a workplace where there is little and infrequent use of vibrating equipment.
- F-HS45D Vibrating Equipment Register - this form can be used to prepare a register detailing all vibrating equipment. Make / model, equipment type, vibration level and maximum daily operating time can all be logged allowing this to be a reference when planning any activity using vibrating equipment.
- F-HS45E HAVS Medical Screening Form - this form can be used to complete a medical screening with a worker prior to them being asked to use vibrating equipment. The purpose of the form is to collect past history of using vibrating equipment and determine if have any symptoms of HAVS or other vibration related issues or issues that could be exasperated by exposure to vibration
- F-HS45F HAVS Register - this form is a combination of the F-HS45C and F-HS45D so the information about vibrating equipment including max safe operating time is detailed on the same form and available to those planning or completing work with vibrating equipment and need to know safe limits for use of equipment
- F-HS45G Hand-Arm Vibration Job Record Form - this form is intended to be used at a job site location or for a single job and can be used to detail all vibrating equipment on site including vibration level and then also and equipment use record to assist with work planning, job rotation and monitoring that no workers are over the ELV limit.
- F-HS45H HAVS Medical Monitoring Form - this form is similar to F-HS45E but is intended for review and monitoring with own workers who are already using vibrating equipment and can be used to determine if any issues
