This example risk assessment has been prepared using the
F-HS20 General Risk Assessment form template - a purpose-built template designed to focus entirely on what matters: identifying hazards, rating risk and recording controls, without any unnecessary fields getting in the way.
The file is supplied in fully editable MS Word format and can be easily customised and saved to Google Docs format if required. The document uses pre-defined style formatting throughout with styles to match the risk-ratings which are input using select boxes pre-populated with all possible risk ratings. All text is in Calibri font for improved readability.
The following document labelling is included in the footer:
- Form number and title
- Information-classification
- Version
- Page number / total pages

Use of Work Equipment Risk Assessment
What Does This Work Equipment Risk Assessment Cover?
- Contact with dangerous parts of machinery — unguarded cutting, rotating, shearing, crushing or nipping parts causing lacerations, crush injuries or amputation
- Ejection of materials — chips, swarf, sparks or broken components ejected at high velocity causing eye injuries and lacerations
- Unexpected start-up during maintenance — machinery starting unexpectedly during maintenance causing entanglement with dangerous parts
- Equipment failure from poor maintenance — defective or poorly maintained equipment causing unexpected failure and contact injuries
- Noise and vibration from work equipment — prolonged use causing noise-induced hearing loss (NIHL) and hand-arm vibration syndrome (HAVS)
- Inadequate training and supervision — use of equipment by untrained or unsupervised operatives
Each hazard includes an inherent risk rating ranging from Moderate to Substantial, and a comprehensive listing of applicable control measures covering areas such as;
- Machine guarding, lock-off/tag-out (LOTO) procedures and PUWER inspection records
- Deflector guards, eye protection and bystander screens for ejection risks
- Isolation of all energy sources before maintenance; permit-to-work where LOTO is not practicable
- Pre-use inspections, planned preventive maintenance and correct tooling selection
- Noise and vibration risk assessments, low-vibration equipment selection and health surveillance
- Operator training records, supervised operation periods and safe systems of work
A residual risk rating is then assigned to each hazard, demonstrating how effective risk management reduces overall risk to a tolerable level.
