Hazard and Risk Assessment Register Template
A hazard and risk assessment register template in Microsoft Excel format for listing the activities and areas of your organisation, the hazards each one carries and who is at risk, scoring the risk before and after controls, and keeping track of the risk assessments behind it. A Google Sheets version is included.
Risk assessment is a legal duty for every employer, and ISO 45001 requires hazards to be identified and the risks from them assessed on an ongoing basis. The register is the overview that ties the two together - the whole picture of hazards across the organisation, and which risk assessment deals with each one. It is one of the first documents asked for when hazard identification is looked at during ISO certification audits.
What's Included in this Hazard and Risk Assessment Register
The workbook is laid out as separate sheets, each doing one part of the job:
- Hazard Review - a listing of typical workplace hazards, such as falls from height, manual handling and slips and trips, each with a short description. Mark whether each one applies to you and give it a risk rating. Anything marked N/A is shaded out and higher ratings are highlighted.
- Activities and Hazards - the main listing. For each activity or area: the hazards and persons at risk, Likelihood and Consequence drop-downs that set the inherent risk rating automatically, the control measures and risk assessments in place, a residual risk rating, the relevant legislation and guidance, and notes on past incidents, reviews and opportunities.
- Risk Assessment Register - every risk assessment with its rating, who completed it, the date, how often it is reviewed and when it is due for renewal, highlighted once the renewal date has passed.
- Instructions - the risk level indicator (the likelihood and consequence matrix behind the ratings) and the review frequencies used in the drop-downs, which you can edit to suit your business.
From Hazard to Risk Assessment
The register works in the same order as the assessment process. The hazard review makes sure nothing obvious has been missed. The activities sheet sets out where each hazard arises and how serious it is before and after controls. The risk assessment register then shows that each significant hazard has an assessment behind it, and that the assessment is being kept up to date.
Because the ratings are colour coded, a Substantial risk with no assessment against it, or an assessment that is overdue for review, is easy to spot - and those are the entries to deal with first.
Who Needs a Hazard Register?
Any organisation implementing or maintaining an ISO 45001 management system, or an integrated system that includes it. It is just as useful without certification in view - any business with more than a handful of risk assessments needs one place to see them all, know when each is due for review, and answer the health and safety questions in tender and supplier questionnaires.
