General Risk Assessment Template

The general-purpose risk assessment for any activity, area or process. It records each activity and its associated hazards and consequences, the inherent risk rating, the control measures in place and the residual risk rating, and confirms whether the residual risk is at an acceptable level.

The Management of Health and Safety at Work Regulations 1999 require a suitable and sufficient risk assessment for work activities. This form gives you a clear, repeatable way to meet that duty and supports the hazard identification, risk assessment and operational control requirements of ISO 45001.

Two Versions in One Download

This product includes two versions of the general risk assessment, so you can use whichever suits the job:

  • F-HS20A - the clean general risk assessment, supplied with a separate Appendix I hazard identification prompt sheet you can work through first
  • F-HS20B - the same assessment with the hazard-category prompt and a full safety-sign reference (prohibition, warning and PPE signs) built into the document

Built for ISO 45001 and the Risk Assessment Duty

The assessment supports the hazard identification, risk assessment and operational control requirements of ISO 45001 (clauses 6.1.2 and 8.1):

  • Each activity described with its associated hazards and potential consequences
  • Inherent risk rating recorded before controls, residual risk rating after
  • The control measures currently in place set out against each hazard

The form is set out so an assessment runs logically from scope to sign-off:

  • Header - completed by, date completed, risk assessment number, the location, task or activity, who is affected or at risk, and persons consulted
  • Assessment table - numbered rows describing each activity, hazard and consequence, with inherent risk rating, control measures in place and residual risk rating
  • Acceptability confirmation - a clear statement that residual risk is acceptable for all activities assessed, or the actions taken or required if not
  • Additional controls required - further actions arising from the assessment
  • Notes - other opportunities to reduce risk and feedback from interested parties
  • Hazard prompt and safety signs (F-HS20B / Appendix I) - a hazard-category checklist and prohibition, warning and PPE sign reference
