Harassment Risk Assessment Form Template

A harassment risk assessment that works through the factors that raise the risk of harassment - public-facing work, lone working, alcohol on or near the premises, and the potential for sexual harassment - and checks the policies, reporting routes and support that should be in place, with risk ratings and actions.

The Worker Protection (Amendment of Equality Act 2010) Act 2023 places a duty on employers to take reasonable steps to prevent sexual harassment of their workers. This form gives you a structured way to assess that risk and evidence the steps taken, and supports the hazard identification requirements of ISO 45001.

Built for the Preventative Duty and ISO 45001

The assessment supports the risk assessment requirements of ISO 45001 (clause 6.1.2):

  • The situational factors that raise harassment risk, each marked applicable or not
  • A risk rating and the controls already in place recorded against each factor
  • Checks on harassment policies, reporting processes and available support
  • A record of concerns identified and further action required

What's Included in this Harassment Risk Form

The form is a structured assessment with clear action prompts:

  • Header - completed by, date and the areas or activities covered
  • Factors for consideration - each factor with applicable Y / N, risk rating, controls in place and notes / actions required, covering public-facing work, communications, lone working, alcohol, policies, reporting and support
  • Concerns and actions - concerns identified and further action required

Who Needs a Harassment Risk Form?
