Risk Assessment Hazard Prompt Template

A prompt sheet that walks an assessor through the common workplace hazard categories so none are overlooked, then provides space to describe each activity and hazard, rate the inherent and residual risk, and record the control measures in place.

Hazard identification is the starting point of the risk assessment duty under the Management of Health and Safety at Work Regulations 1999, and it underpins the hazard identification and risk assessment requirements of ISO 45001. This form gives you a simple, repeatable prompt so assessments start from a complete picture.

The form supports the hazard identification and risk assessment requirements of ISO 45001 (clause 6.1.2):

  • A prompt list of common hazard categories - slips and trips, hazardous substances, work at height, biological hazards, struck-by, noise, confined space, vibration, manual handling and more
  • A Yes / No marker and risk rating against each hazard category so nothing is skipped
  • Space to describe each activity and its associated hazard, referenced back to the prompt list
