This form is used to record details of assessments completed on substances that are hazardous to health.

The COSHH assessment allows you to detail trade name/manufacturer (s) in list format and graphics can be used to illustrate the hazard classifications and PPE required. 

You can delete the non-applicable symbols when filling out the form.
