COSHH Assessment Form Template
A single-substance COSHH assessment form for recording the hazards, exposure routes, controls and emergency actions for each hazardous substance used, stored or produced in your workplace. One completed form per substance gives you a clear, consistent record of how exposure is prevented or controlled.
The Control of Substances Hazardous to Health Regulations 2002 (COSHH) require every employer to assess the risks from hazardous substances before work starts. - Hazard and first aid - effects of exposure and first aid treatment for each exposure route
- Safe working exposure limits - space to record workplace exposure limits from the SDS or EH40
