Accident Report Form Template
An accident report for recording workplace accidents and injuries. It captures the injured person, the date and time, the nature of the injury and the activity at the time, whether it was reportable under RIDDOR, whether a safe system of work and PPE were in place and followed, statements from the injured person and witnesses, the cause and recommendations.
Employers must record accidents and report certain injuries under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR), and investigating accidents is central to ISO 45001.
