Health Conditions Policy Template
A health conditions policy template committing to an inclusive workplace that is safe and does not harm workers' health, by monitoring health and wellbeing and checking whether reasonable adjustments are needed.
The policy has Approved By and Date Approved fields and carries a Public information classification, so it can be shared with workers, clients and other interested parties.
What the Health Conditions Policy Covers
- Purpose of screening - whether someone is fit and safe to do the job, and any reasonable adjustments needed
- Medical questionnaire - required once an offer of employment has been made
- Later questionnaires and examinations - where a role or health surveillance requires it, with medical examinations only with consent
- Non-discrimination - the duty not to discriminate against disabled people taken into account
- Health surveillance - based on the risk of harm from work, with general advice on other health matters
- Employees' duties - attending health surveillance and reporting conditions that affect safe working or may be caused by work
- Medical confidentiality - clinical records accessed only by occupational health advisers and the employee, with only fitness for work outcomes kept by the organisation
- Review - at least once a year at management review
Versions Available
The main policy refers to the duty not to discriminate and to data protection law without naming the legislation. P-81 Health Conditions Policy (V2) names the Equality Act and the Access to Medical Reports Act and adds a legislation section, for organisations in the UK that want the references spelled out.
Written to Be Shared
Like every alphaZ policy, this is written as a public policy statement that can be shared openly with workers, customers, suppliers and assessors. The internal detail of how it is carried out belongs in your procedures.
Using the Policy
Add your company name and logo, check each commitment matches what your organisation does and edit any that do not, then approve and date the policy and share it with workers and anyone else who needs to see it.
Who Needs a Health Conditions Policy?
Employers that use medical questionnaires, medical examinations or health surveillance.
Works with the Medical Questionnaire
The F-HR8 Medical Questionnaire Form can be used for the questionnaire the policy asks for, and the F-HR11 Reasonable Adjustment Request for adjustments.
Document Control
The footer carries the policy number and title, information classification, version and page numbers, and the approval fields record who approved the policy and when, so it can be controlled with the rest of your management system documents and listed on a document register such as the F-IMS20 Document Register.
