Driving Company Vehicles Form Template
A driver declaration and briefing form to be completed by every member of staff who drives a company vehicle, or who drives their own vehicle on company business. It records the driver's name, the address and number on their driving licence, and confirms they have read the company driving rules.
Driving for work is treated as a work activity under health and safety law, so the same duty to manage risk applies on the road. This form gives you a signed, dated record for each driver and a clear summary of the rules they are agreeing to follow.
Two Versions in One Download
The download includes two versions of the form, so you can use the one that suits how you manage your drivers:
- F-HR2 - a straightforward driver declaration and rules briefing
- F-HR2A - a fuller version that also records any previous driving convictions, licence endorsements and fines
Built for Managing Occupational Road Risk
The form captures what you need to show each driver is authorised and informed:
- Driver name and the address and number shown on their driving licence, so a copy can be checked and held on file
- A summary of the company driving rules, from licence and insurance requirements to reporting accidents, endorsements and defects
- Clear statements on drugs and alcohol, mobile phone use, roadworthiness checks and personal use of company vehicles
- A space (F-HR2A) to record previous convictions, endorsements and fines as part of the check
What's Included in this Driving Company Vehicles Form
The form is set out so a driver can be recorded and briefed in one place:
- Driver details - name, and the address and number on the driving licence
- Information for drivers - a summary of the main points of the company driving policy for the driver to read and follow
- Previous convictions (F-HR2A) - details of any driving offences, endorsements or fines
- Declaration - the driver's confirmation that they hold a valid licence and will follow the rules
Who Needs a Driving Company Vehicles Form?
Any organisation whose staff drive company vehicles, or use their own vehicles for work, needs a record that each driver is licensed, insured and aware of the rules. It suits trades, service and delivery businesses, care providers and any employer with pool cars or vans, and supports the occupational road-risk controls expected under ISO 45001.
Included in the ISO 45001 Toolkit
The F-HR2 form is part of the ISO 45001 Toolkit - the forms, registers, procedures and guidance for running a health and safety management system in one download - and is included in every alphaZ ISO toolkit that covers managing health and safety.
