Form for reviewing and documenting the risk of harassment to workers.

The form includes a section to detail an overview of the assessment including details of who has completed the form, date and areas covered.

The form then uses a table layout with input fields to detail;

• Factors for consideration - pre-populated content
• Applicability - select box
• Risk Rating - select box
• Controls in place - text
• Notes / comments / actions required - text

This layout is pre-populated with a number of factors and this layout allows the register to capture that is risk factor is not-applicable.

The following factors are included on the default form;

• Public-facing areas or visitors
• Communications – taking calls or using other communication systems
• Lone working
• Alcohol consumed on premises or in surrounding area
• Effective policies and procedures in place regarding harassment
• Effective harassment reporting process in place
• Support available in the case of harassment
• Potential for sexual harassment to occur

The form then has a section at the end to summarise any concerns or further actions required.
