Driver FORS Declaration Form Template
The F-FORS1 Driver FORS Declaration is a Word form that records each driver's confirmation that they understand their individual responsibilities, as set out in the company's FORS (Fleet Operator Recognition Scheme) policies, the Highway Code, the FORS Manual, company handbooks, their employment contract, the company management system and the risk assessments for driving work.
The driver signs and dates the declaration at the foot of the form, giving the operator a signed record for each driver that can be kept on file and shown at a FORS audit.
What's Included in this Form
- Driver responsibilities - a statement that all drivers need to be aware of their responsibilities as set out in the documents listed
- FORS policies - health and safety, complaints, transport infringements, serviceability and roadworthiness, load safety, tyre management, driving standards, staff induction, in-vehicle communication, health and eyesight, working time and drivers' hours, fuel, emissions and air quality, road traffic collisions, passenger safety where relevant, specialist operations, operational security and counter terrorism
- Other documents - the Highway Code, the FORS Manual and FORS guidance, company handbooks including the drivers handbook, the employment contract, and the company management system with its policies, forms and procedures
- Risk assessments - lone working, manual handling, hazardous substances, driving and deliveries, slips, trips and falls, loading and unloading, reversing, manoeuvring and turning, access to vehicles and working at height, working around moving vehicles, coupling, uncoupling and towing, and in-vehicle technology
- Declaration - the driver's name, a declaration that they understand their responsibilities, and their signature and date
Who Needs a Driver FORS Declaration?
Fleet operators holding or working towards FORS accreditation that want a signed record showing each driver has been told which policies, risk assessments and documents apply to them, at induction and again when the documents change.
Included in the FORS Document Compliance Toolkit
This form is part of the FORS Document Compliance Toolkit, alongside the FORS policies, the drivers handbook and the FORS registers.
Using the Form
Edit the lists so they name only the policies, handbooks and risk assessments your drivers actually work to, then ask each driver to read them, sign and date the declaration. File the signed form with the driver's records and ask for a new one when a listed document changes.
Choosing Between FORS1 and FORS2
F-FORS1 covers the driver's responsibilities only and is classified for business use. The F-FORS2 Driver FORS Declaration - Combined adds collisions, endorsements and health questions in a single confidential form, for operators who want one declaration to cover everything.
Document Control
The footer carries the form number and title, the issue number, page numbering and the information classification, Business Use, so completed forms can be controlled and filed with the rest of your FORS records.
Related Documents
The documents the declaration refers to include the P-FORS Policies, the DRI1-1 Drivers Handbook and risk assessments such as RA-HS39 Driving and Deliveries. The F-IMS33 FORS Review Register checks the declarations are in place.
