Accident and Incident Reporting Training Talk

A training talk on accident and incident reporting for all staff - making the area safe, reporting accidents, incidents and near misses, and how investigations lead to corrective action. It is a one-page attendance record with the talk description already filled in, and it refers to the guidance document to run the talk from.

The Talk and the Documents It Refers To

  • TT-7-04 Accident and Incident Reporting - the attendance record, with the topic and talk description ready to use and a signature line for each attendee
  • GG-7-04 Accident and Incident Reporting - the guidance document the talk is based on, to prepare from and to issue to staff afterwards

Talk Description

  • Making the area safe first, before any investigation
  • Reporting every accident, incident, near miss and work-related ill health
  • Injuries, diseases and dangerous occurrences that must be reported to the authorities
  • Time off work after an accident, and accidents to members of the public
  • Investigating accidents and incidents, and finding the cause
  • Corrective action, and sharing what has been learned

Running the Talk

Read through the guidance beforehand, talk the points through with the team, then have everyone sign the record. Keep the signed record as evidence that the briefing took place and who received it.

Why Near Misses Are Reported Too

A near miss is the same chain of events as an accident, without the injury. The talk asks for near misses to be reported so that the cause can be found and fixed before someone is hurt.

Adapting the Talk

The record and the guidance are both fully editable, so add how incidents are reported on your site, where the accident book or reporting form is kept, and the names of your first aiders and the person who investigates. Update the talk description if you cover extra points.

Keeping the Training Record

File the signed record with your training records, or log the talk against each attendee in a training matrix, so you can show who has been briefed and when.

Who Is It For?

All staff, including supervisors who record and investigate incidents. It works at induction, as a refresher and after an accident or near miss on site.
