This risk assessment identifies the key hazards associated with confined space entry including;
  • Oxygen deficiency and toxic atmosphere — depletion of oxygen or accumulation of toxic gases (methane, hydrogen sulphide, carbon monoxide, carbon dioxide) causing incapacitation and death
  • Engulfment and drowning — sudden inrush of liquid, gases or free-flowing solids engulfing the entrant causing asphyxiation
  • Fire and explosion within confined space — ignition of flammable atmospheres by hot work or electrical equipment causing blast and burn injuries

Each hazard includes an inherent risk rating of Substantial, and a comprehensive listing of applicable control measures covering areas such as;

  • Confined space risk assessment and permit-to-work completed before any entry
  • Continuous atmospheric monitoring with calibrated multi-gas detector before and during entry
  • Forced ventilation to maintain safe oxygen levels and disperse contaminants
  • Trained standby person in constant attendance; emergency rescue plan with non-entry retrieval system
  • Safety harness and lifeline worn by all entrants; all pipework and inlet connections isolated before entry
  • Intrinsically safe electrical equipment only; hot work permit required for all hot work in confined spaces
  • Minimum two-person team; no lone working in confined spaces

A residual risk rating is then assigned to each hazard, demonstrating how effective risk management reduces overall risk to a tolerable level.
