Design Change Request Form Template
A design change request form for capturing a proposed change to a design, product or part and controlling how it is reviewed. It records who raised the change, the project, product or part number affected and the details of the suggested change, so nothing is altered without a documented request.
ISO 9001 requires changes made during design and development to be identified, reviewed and controlled (clause 8.3.6). This form gives you a simple, repeatable way to raise and record a design change so it can be assessed before it is made.
Built for Controlled Design Changes
The form captures what a controlled design change needs, supporting clause 8.3:
- Who raised the change and when
- The project, product or part number affected
- A clear description of the suggested change
- A documented request that can be reviewed before the change is made
What's Included in this Design Change Request Form
The form is a single, focused request:
- Header - raised by and date
- Reference - the project, product or part number the change relates to
- Details of suggested change - a full description of what is proposed
Who Needs a Design Change Request Form?
Any organisation that designs or develops products and needs to control changes to them. It suits manufacturers, engineers and product developers, and supports the design change control requirements of ISO 9001.
Included in the ISO 9001 Toolkit
This form is part of the ISO 9001 Toolkit - the forms, registers, procedures and guidance for running a management system in one download - and is included in every alphaZ ISO toolkit that covers quality management.
Pairs with the Design Review and Planning Form
Design changes sit alongside the wider design process on the F-Q1 Design Review and Planning form, so a design is controlled from first plan through to change.
