Employee PPE Issue Sheet Template
A per-employee PPE issue sheet for recording the personal protective equipment issued to one individual. It lists the quantity and description of each item, the date issued and a signature confirming receipt, giving a clear record on the employee's own file.
The Personal Protective Equipment at Work Regulations 1992 (as amended 2022) require suitable PPE to be provided free of charge where risks cannot otherwise be controlled. This sheet gives you a per-employee record of PPE issued and supports the operational control requirements of ISO 45001.
