PPE Issue Register Template

A register for recording all personal protective equipment issued across the organisation. It logs the employee, the quantity and description of PPE issued, the date and a signature confirming receipt, so there is a single, signed record of what has been provided.

The Personal Protective Equipment at Work Regulations 1992 (as amended 2022) require suitable PPE to be provided free of charge where risks cannot otherwise be controlled. This register gives you the evidence that PPE has been issued and supports the operational control requirements of ISO 45001.
