HR form for employees to document hours worked.
This form is a fairly simple 1 page timesheet where employees enter their name / location and then a table format for logging hours with the following column headings;
- Date
- Start Time
- End Time
- Notes / Comments
- Total Hours
There is then a total for the timesheet, a large box for overall Notes / Comments and a declaration for the member of staff to confirm that details submitted are correct (details of action if incorrect details supplied can be added here).
