 HR form for employees to formally report a grievance.

The form is split into 5 sections;

  • Section 1 - Report Overview - this section is to be completed by the person reporting the grievance and collects their details, who they wish the grievance to be submitted to (in case it is not the line manager as the grievance may be about them) and overview of the grievance. Also collects what action they are wanting in case the reporter is only wanting their grievance logged but don't want a meeting or full investigation. A declaration is included to confirm information provided is correct and accurate. If a meeting is requested a section for detailing name of a companion who is to attend the meeting is included.
  • Section 2 - Report Review - this section is marked as to only be completed by the reviewer (can easily be split off into a separate form if preferred)  and includes sections for detailing review of the grievance, cause and recommendations / comments. A Yes / No select box section is included for detailing if any further action required. 
  • Section 3 - Action Taken - this section allows details of the review and investigation to be logged including a section for outcome, action taken and details about action taken to inform the reporter and a section for details of any appeal. Guidance on redacting names  (for data protection) if sharing the form with the reporter is included.
  • Appendix i - Statements - this section is for logging witness statements completed as part of any investigation
  • Appendix ii - Meeting Notes - this section for logging details of any meetings held including details of date, location and attendees.

This form is designed to be consistent with the grievance procedure as detailed in the General Staff Handbook.

A Record Retention Summary is included at the end of the form which can be used to provide on-form summary of where the completed form should be filed, how protected and details of sharing and retention for each section of the form to ensure compliant with personal data protection legislation.
