This form is for checking and documenting that required checks and screening are completed for employees.
If you are taking on new employees it is important that there is a clear process to be followed to ensure any mandatory and other checks are completed correctly.
This form includes the following sections;
- 1. Identity and right to work - details of what is checked including passport, address evidence, details for payroll, legal right to work, qualification, licenses, confidentiality / non-disclosure agreement
- 2. Screening / Background Checks - summary of other checks including criminal record, financial, employment and social media
- 3. References / Past Employment Checks - summary of reference checks
- 4. Driver Checks - details of driving licence and driving record
- 5. Medical Checks - overview of any medical checks required or completed including drug / alcohol testing
- 6. Additional Checks - section for adding any further checks required
- Review and Authorisation - section to be completed to confirm that all checks completed and overall outcome
This form includes a 'Record Retention Summary' section so details of protection and retention of the form once it is a completed record are detailed right on the form without any need to check personal data register for retention period.
