Update Details: The Supplier Contractor Appraisal form updated, with two new versions adding ethical behaviour and sustainability questions for suppliers, and a new Ethical Behaviour Assessment form in light and full versions.
Supplier Contractor Appraisal Form Updated
The one-page supplier appraisal gathers a supplier's details, certifications, memberships, licences and insurance, enforcement history and legislative compliance in one place before they are approved, with an office section to record the assessment and whether they go on the key suppliers list. This update adds:
- Enforcement - bribery or corruption, and employment-related enforcement such as for minimum wage or illegal working, alongside the health and safety, environmental and data breach questions
- Legislative compliance - environmental protection and data protection, alongside health and safety, labour standards, equal opportunities and anti-bribery
- Supplier sign-off - completed by, position and date
The layout has also been refreshed.
New Supplier Appraisal Forms - Ethical Behaviour and Sustainability
Two new versions of the supplier appraisal keep the same first page and add questions for the supplier, answered Yes, No or N/A, with a details box for anything they cannot confirm.
- F-Q9A - adds 10 ethical behaviour questions, covering a code of conduct, workers working freely and of legal working age, freedom to join a trade union, bullying and harassment, raising concerns without retaliation, standards set for their own suppliers, protecting confidential information, accurate claims, and prompt notice of any breach affecting what they supply
- F-Q9B - as F-Q9A, plus 8 sustainability questions on environmental policy, measuring energy use and carbon emissions, plans to reduce emissions, waste and packaging, preventing pollution, environmental impact when buying, environmental information about what they supply, and the effect on the local community and biodiversity
F-Q9, F-Q9A and F-Q9B are all downloaded from the same page;

New Ethical Behaviour Assessment Forms
The ethical behaviour assessment looks at the organisation itself, or a branch or department, rather than a supplier. Both versions record the company or location, who completed it and when, and the scope and basis of the assessment, with each question answered Yes (in place and evidenced), Partly, No or N/A.
- F-Q142A - a one-page light version with 16 questions and a comments column, from written values and leadership by example to bribery, fair pay and treatment, customer data, supplier standards, environmental impact and awareness at induction
- F-Q142B - a full version with 47 questions in 10 sections and an evidence column, covering leadership and governance, honesty of information, raising concerns, bribery and conflicts of interest, people and working conditions, customers and fair competition, the supply chain, environment, community and tax, human rights and technology, and awareness and training
Both end with a comments and actions box, reviewed by and date. The questions draw on the UN Global Compact principles, the UN Guiding Principles on Business and Human Rights, the OECD Guidelines for Multinational Enterprises on Responsible Business Conduct and ISO 9001:2026 clauses 5.1.1 i) and 7.3 e).
