Hand-Arm Vibration Policy Procedure Template
The Hand-Arm Vibration Policy Procedure is a Word template that sets out how a business manages exposure to hand-arm vibration (HAV) at work. It covers identifying and assessing the risk before work starts, control measures, health surveillance and training, the records to keep, monitoring and review, and a guidance box giving the daily exposure action and limit values.
It has Title, File Location, Issue No, Issue Date, Controlled By and Approved By fields, an Amendment Record and a Business Use information classification in the footer, and a scope section for the business to complete with the operations, work areas and workers it covers.
What the Policy Covers
- Amendment record, summary, scope and applicability - the control note on amendments, the purpose of the policy, a scope to complete with the operations and workers covered, and who must follow it
- Identification and assessment of risk - identifying equipment and tasks that expose workers to vibration, and a risk assessment before work using duration and vibration data
- Control measures - training, an equipment register or labels with vibration levels and safe operating times, low vibration tools, job rotation, maintenance, PPE, timers and reporting discomfort
- Health surveillance - enrolling at risk workers in health surveillance, training on risks and symptoms, and the screening, usage and manufacturer records to keep
- Monitoring and review - supervisors checking controls and reviewing procedures annually or after incidents, and extra controls when the volume of equipment use changes
- Guidance box - points for ongoing safe use, including health surveillance at or above the action value and the daily exposure action and limit values
Built for Managing Vibration Exposure
- A risk assessment before work starts, using equipment type, duration of use and manufacturer or recorded vibration data
- Vibrating equipment listed on a register or labelled with its vibration level and maximum safe operating time
- Health surveillance for workers identified as at risk, with screening records kept
- Records of how long each person uses vibrating equipment on certain tasks
- Annual review by supervisors, and a review after any incident or change in equipment or process
Who Needs a Hand-Arm Vibration Policy Procedure?
Any business whose workers use vibrating tools or equipment, and the supervisors responsible for planning that work, controlling exposure and keeping surveillance and usage records.
Using the Policy
Complete the scope with the operations, work areas and workers it covers, and edit the control measures and records to match the equipment you use. Name the person in the Controlled By field, give it top level approval and brief operatives and supervisors.
