RPE Monthly Inspection Template
A monthly inspection record for respiratory protective equipment (RPE). It identifies the device and who it is issued to, then records the routine checks - cleanliness, filters, seal and facepiece, straps and fittings, valves and visor - month by month, with a competent-person sign-off that the device is safe to use.
The Control of Substances Hazardous to Health Regulations 2002 require RPE to be maintained in efficient working order and good repair, and HSE guidance HSG53 sets out the checks. This form gives you that maintenance record and supports the operational control requirements of ISO 45001.
