This form is for logging emergency contact details for an employee.

As well as the employee contact details the contact details for emergency contacts are listed in individual table format so the form can be easily expanded to increase the number of contacts listed (the default form includes 2 emergency contacts).

This form also includes a 'Record Retention Summary' section so details of protection and retention of the form once it is a completed record are detailed right on the form without any need to check personal data register for retention period.
