Hand-Arm Vibration Audit Checklist Template

A process audit checklist for auditing the management of hand-arm vibration (HAVS), covering policy and responsibilities, identifying vibrating tools and tasks, risk assessment and exposure, reducing exposure, maintenance, and medical screening. Many of the questions carry a note on what to check. It is set out on the F-Q2 Audit Checklist format, with the scope, criteria, auditor and date recorded at the top, and each question alongside the auditee and the evidence viewed, response and findings.

Questions Covered

  • Whether there is a defined HAVS policy and procedure, with management visibly committed to preventing HAVS
  • Whether roles and responsibilities for managing vibration are assigned and communicated
  • Whether all tasks and equipment that expose people to vibration have been identified
  • Whether vibration risk assessments are up to date and take account of the tool, how long and how often it is used, and individual susceptibility
  • Whether exposure is measured or estimated and recorded, and compared with the exposure action and limit values
  • Whether alternative methods, low-vibration tools and vibration-reducing accessories are used where possible
  • Whether tools are maintained and inspected, with faulty equipment taken out of use
  • How job rotation, exposure time limits and each person's cumulative exposure are managed
  • Whether medical screening is completed before people work with vibrating equipment, and kept up

Carrying Out the Audit

Check the exposure records against the tools seen in use and the time people actually spend on them, which shows whether the estimates reflect the work. Asking an operative how long they have used a breaker or grinder today, and comparing it with the planned limit, tests whether exposure is really being managed.

Planning the Audit

Have the HAVS policy, the vibration risk assessments, exposure records, tool maintenance records and medical screening records to hand, and plan to see vibrating tools being used.

Part of the Process Audit Checklists

This is one of the alphaZ process audit checklists, each covering a single area of the management system, so audits can be spread across the year using the ER11 Audit Schedule. The completed checklist becomes the record of the audit, and the results can be written up using the F-Q31 Audit Report.

Adapting the Checklist

The questions can be edited, added to or removed to suit the way the organisation works, and the scope and criteria at the top changed to match the area being audited. Because every alphaZ audit checklist uses the same F-Q2A layout, audit records look the same whichever area was audited, which makes them easier to review and compare.

Using the Findings

Exposure that is not being recorded or controlled, or missed medical screening, is raised as an action and dealt with before the work continues.
