A worker read and understood form for recording that an employee has received, read and understood the documents that apply to them - the management system, policies and handbooks, risk assessments, method statements, safe operating procedures and company rules. Each item is ticked as received and the worker signs a declaration.

Awareness is a requirement of every management system - people have to know the policies and procedures that apply to them, not just be handed them. This form gives you documented proof that key documents were received, read and understood.
