This form includes sections for the following information;

- Declaration with statement confirming all details supplied are correct
There are 4 different versions of the form available for download;
- F-HR1 Employee Personal Details Form - the default version on the form includes all the sections detailed above
- F-HR1A - this version uses bold section indicators to split the form into sections and includes a Record Retention Summary so can state on the form the retention period and protection for completed records
- F-HR1B - this version is as per A but the Emergency Contact Details section is not included
- F-HR1C - this version only includes the Emergency Contact Details section
The different versions of the form are to support good practice and data protection compliance relating to retention of personal data. As some content, such as emergency contact details will not be retained after an employee leaves the organisation it is held separately to ensure it is easier to delete.
To support correct record retention the A, B & C versions of the form include on-form record retention details to provide a clear summary of retention and protection for all sections and personal data included on the form(s).
