The ER18 Accident Statistics document is a register template which may be used to record accident data and to review and summarise accident statistics. In particular the register can be used to generate the All Accident Frequency Rate (AAFR) and the Lost Time Incident Rate (LTI).

The user’s accident data is to be entered into the accident data sheet – (only summary data, no personal details) including Year, Date, Type (controlled entry field), Category (controlled entry field), summary/ description, total days lost and notes.

Automatic summary sheet provides overview of data for each year and additional fields allow total staff and total hours worked to be entered and this is used to calculate and return a frequency value for each year.

Template also includes calculations for AAFR (All Accident Frequency Rate) and LTI (Lost Time Incidents).

Lost Time Incident Rates are calculated using OSHA guidance (US) and are relevant for compliance with accident reporting regulations. 

The reporting format will also help with compliance for the Health and Safety Executive (UK). Both accident and RIDDOR (Reporting of Injuries Diseases and Dangerous Occurrences) statistics are required for Acclaim Accreditation by Constructionline.

Analysis sheet also included to provide graphical overview of data including Accidents / Near Misses by year and Accidents / Near Misses by category.

Category and type can be entered on the setup sheet.

This register can be used to summarise details of accidents / near-misses and if data for total hours worked also added will provide a 'frequency' value which is a measure of how many accidents happen for each hour worked - this can be used as a measurable health & safety objective.

Various schemes require submission of accident data and this register allows this to be easily compiled and supplied when required.
